Please click on and complete these new client history and intake forms:  Part 1 (couples need each complete Part 1 only) & Part 2 prior to our first meeting.  Some of the questions may seem redundant but I appreciate you completing them as thoroughly as you can.  If you'd like you can send it via email attachment or (please don't fold pages) US Mail; or bring it with you to our session.  I've kept the forms as short and succinct as possible for your convenience and to reduce waste; please use 2-sided printing if you have this option.
